Boy, un garçon qui promet d’obtenir vengeance après que sa famille ait été décimée par Hilda Van Der Koy, matriarche dérangée d’une dynastie post-apocalyptique corrompue l’ayant laissé sourd et muet. Mené par sa voix intérieure inspirée du narrateur du jeu vidéo favori de son enfance, Boy est entraîné par un mystérieux shaman afin qu’il se transforme en une arme mortelle, puis mette en marche son plan destructeur la veille du carnage annuel des dissidents. Le chaos frappe lorsque Boy fait l’étalage de ses talents en arts martiaux, provoquant un bain de sang. Alors qu’il tente de retrouver ses repères dans ce royaume complètement déjanté, Boy se joint malgré lui à un groupe de rebelles, tout en devant composer avec le fantôme belliqueux de sa petite sœur.
